Hormone replacement therapy for men addresses what TRT alone may miss. A comprehensive HRT panel covers thyroid (free T3, free T4, reverse T3, TSH, antibodies), adrenal (AM/PM cortisol, DHEA-S), and the full sex-hormone profile (total T, free T, sensitive estradiol, SHBG, prolactin). The premise: low energy or low libido isn't always testosterone — it might be subclinical hypothyroidism or chronic cortisol dysregulation.
Look for: comprehensive baseline labs (not just total T), documented prescriber credentials, clear dosing rationale, estradiol management as part of the plan, follow-up labs at 6–12 weeks, and pricing transparency. Safer is the wrong frame. Bioidentical hormones are molecularly identical to what your body produces (testosterone cypionate is bioidentical in this sense) and may have different side-effect profiles, particularly for estradiol and progesterone. Current evidence does not show TRT increases cardiovascular events in men with documented hypogonadism receiving properly monitored treatment — the TRAVERSE trial (2023) was the largest study to date and found no increased cardiac event rate vs placebo. That said, TRT can elevate hematocrit, which clinics monitor periodically and address through dose adjustment or therapeutic phlebotomy. Long-term safety depends on the monitoring protocol, not just the medication.
