Two barrels, one number, a hundredfold apart

A syringe measures volume. That is the only thing it does, and everything printed on the barrel is a way of describing volume — but not the same way on every barrel, which is where this goes wrong.

An insulin barrel is numbered in UNITS. U-100 means the scale is laid out so 100 units is exactly 1 mL, so one printed unit is 0.01 mL and nothing else. That is a definition of the scale, not a measurement of a product. A luer-lock barrel is numbered in MILLILITRES: the numbers on it are the volume directly, so 0.5 on a 1 mL luer-lock is half a millilitre.

Both of those barrels come in a 1 mL size. They are the same length, they hold the same volume, and they sit in the same drawer. And the instruction "draw 0.5" is accepted by both without complaint: 0.5 mL on the luer-lock, 0.005 mL on the insulin barrel. The plunger stops at a printed 0.5 in both cases. Nothing warns anybody. That is 100 times the dose in one direction and 1% short in the other, and the person who made the mistake did everything right except identify the instrument.

This is not a hypothetical error class. The FDA label for HUMULIN R U-500 warns patients directly: "You can give yourself the wrong dose if you use any other syringe, such as a U-100, tuberculin or allergy syringe" and "Never use other syringes. The lines and numbers on other syringes will not measure your dose correctly." A tuberculin syringe is the 1 mL barrel numbered in millilitres — the label is naming exactly this confusion, in a patient leaflet, because it happens.

And there are more than two scales. The same label describes a third: "The U-500 syringe has a green U-500 symbol and a green Needle Shield on the syringe", and "Each line on the U-500 syringe measures 5 units of U-500 insulin" — a barrel whose marks mean something different again, quantified by the label as a "5 times overdose" if a U-100 barrel is used instead. Veterinary U-40 barrels are a fourth, and the insulin syringe conversion chart covers those in full. This page stays with the two you are most likely to have in the house at once.

Once you know which family you are holding, the second question is which size, and the answer is the smallest that fits. Not because a larger one is inaccurate — the volume is the volume — but because the same draw occupies more of a small barrel and is therefore easier to set. 0.5 mL is 100% of a 0.5 mL insulin barrel, 50% of a 1 mL one, and 17% of a 3 mL luer-lock. Setting a plunger to a tenth of the way along a long barrel is a harder piece of eyesight than setting it halfway, and it is harder again because nobody publishes how far apart the lines are.

Which is the last thing worth saying here, and it is a refusal. This page does not tell you the graduation interval on your barrel, because we could not source it. Searching manufacturer instructions-for-use, FDA device listings and 510(k) summaries across embecta/BD, UltiMed, Nipro, Exelint, HTL-Strefa, B.Braun, Medline, Cardinal and Terumo turned up exactly one statement: embecta's, that its range is "Designed with large dose unit markings, with ½ unit scale marking available on the 0.3mL barrel". Every confident "1 unit on a 0.5 mL, 2 units on a 1 mL" figure in circulation traces back to patient handouts and SEO pages rather than to labelling. So the chart below prints "Not documented" and asks you to hold your own barrel up and count the lines between two numbers. /charts/insulin-syringe-conversion-chart says the same thing, and the code behind this page returns the same answer, so the site cannot start contradicting itself on it.

The formula

on an INSULIN barrel (numbered in units):

volume (mL) = number set ÷ 100

number set = volume (mL) × 100

on a LUER-LOCK barrel (numbered in millilitres):

volume (mL) = number set

the same numeral on both:

ratio = 100× — luer-lock reading over insulin reading

barrel fill (%) = volume ÷ barrel capacity × 100

The 100 is the definition of a U-100 barrel, not a measurement of one. Note that the conversion on a luer-lock barrel is not a conversion at all — the number IS the volume — which is why an instruction written for one and read on the other is out by the whole factor rather than by a bit. The fill figure is the argument for the smallest barrel that holds the draw: it does not change the accuracy of the volume, it changes how easy it is to set. No graduation interval appears in any of this, because none is documented for four of the five barrels below.

Worked example

A 0.5 mL draw — the volume a 500 IU hCG dose comes to from a 1,000 IU/mL vial, and a common testosterone volume too:

  • On a 50-unit insulin (0.5 mL) it reads 50 units, filling 100% of the barrel
  • On a 100-unit insulin (1 mL) the same volume reads 50 units, filling 50%
  • On a 1 mL luer-lock it reads 0.5 mL — the same volume, a different numeral, because the barrel is numbered in a different thing
  • On a 3 mL luer-lock it also reads 0.5 mL, but fills only 17% of the barrel
  • Smallest insulin barrel that holds it: 50-unit insulin (0.5 mL). Smallest luer-lock: 1 mL luer-lock
  • And the error: told "draw 0.5" and reaching for the insulin barrel, you would set the plunger to 0.5 units — 0.005 mL, 1% of what was meant. Told "draw 50" and reaching for the luer-lock, you would draw 50 mL, which no barrel here holds — the one version of this mistake that stops itself

Which barrel holds it, and what number you set

Read one row across and the point of the page is visible: the same physical volume is called 50 on a 1 mL insulin barrel and 0.5 on a 1 mL luer-lock. The row header names the smallest barrel that holds the draw, which is the one to reach for — a larger barrel measures the same volume just as truly, but a smaller one puts the plunger further along and makes the setting easier to see. Every figure is computed by the same functions the selector above calls.

The same bare number on both barrels

An instruction that says "draw N" with no unit attached. This is what N is worth on each family, and whether anything would stop you if you picked the wrong one.

The fourth column is the finding, and it is not reassuring: across the whole band of numerals an instruction is actually likely to carry, both readings fit a barrel somebody owns, so nothing in the physical world corrects the mistake. Only at the extremes — above 3, where no luer-lock holds the millilitre reading, and above 100, where no insulin barrel holds the unit reading — does the error catch itself. The fix is not vigilance, it is writing: never quote a bare number. "0.5 mL on a luer-lock" and "50 units on a U-100 insulin barrel" are the same instruction and neither can be misread.

How much of the barrel the draw actually uses

The same volume as a percentage of each barrel. This is the whole argument for picking the smallest one that fits: the volume is equally true on all of them, the plunger position is not equally easy to see.

Anything under about a tenth of the barrel is being set against a very short stretch of scale, and since no manufacturer publishes how far apart the lines are on most of these, there is no way to say how many marks that stretch contains. Where a small draw has to go in a large barrel — because the needle or the route demands it — that is a reason to be slower about it, not a reason the number is wrong.

What each one is, rather than what it converts to. The column that matters most is the second: it is the difference that makes a bare number dangerous.

Four of the five cells in the third column read "Not documented" and that is a finding, not a gap we forgot to fill. We could not source the printed graduation interval for any barrel here from a manufacturer instruction-for-use or an FDA device listing; the one statement anybody makes is embecta's, that a half-unit scale is AVAILABLE on the 0.3 mL barrel, which is a claim about what is offered rather than about what is on the barrel in your hand. Count the lines between two numbers on your own barrel before you set a small dose on it. /charts/insulin-syringe-conversion-chart gives the same answer. The needle column is what is commonly supplied, not a recommendation — gauge, length and route belong to whoever is prescribing.

What is the difference between an insulin syringe and a regular syringe?
The scale printed on the barrel, and it is the only difference that can hurt you. An insulin barrel is numbered in units on the U-100 scale, where 100 units is exactly 1 mL, so one unit is 0.01 mL. A luer-lock or tuberculin barrel is numbered in millilitres directly. Both are sold in a 1 mL size. "Draw 0.5" means 0.5 mL on the luer-lock and 0.005 mL on the insulin barrel — a hundredfold difference, with the plunger stopping at a printed 0.5 either way. The other differences follow from use: insulin barrels have a fixed short fine needle, luer-lock barrels take a detachable one you choose.
How many units is 0.5 mL on a syringe?
On a U-100 insulin barrel, 0.5 mL is 50 units — fixed, because U-100 means 100 units to the millilitre. On a luer-lock barrel 0.5 mL is simply the mark labelled 0.5, and it is not "50" of anything. That is the trap in the question: "how many units" only has an answer if the barrel is graduated in units. If someone told you to draw 50 and you are holding a luer-lock, 50 mL is what that instruction literally says and no barrel in ordinary supply holds it.
Which syringe size should I use for my injection?
The smallest barrel of the right family that holds the volume in one draw. A 0.2 mL draw is 67% of a 0.3 mL insulin barrel, 20% of a 1 mL one and 7% of a 3 mL luer-lock — the same volume, but far easier to set on the first. Which family is the right one is a route question, not a volume question: aqueous subcutaneous injections generally go on insulin barrels, oil-based intramuscular injections need the detachable longer needle of a luer-lock, and that choice belongs to whoever is prescribing rather than to a calculator.
Can I use an insulin syringe for an intramuscular injection?
Mechanically, usually not. Insulin barrels carry a fixed needle — embecta supplies 6 mm, 8 mm and 12.7 mm lengths — and 12.7 mm is half an inch, which does not reach muscle through subcutaneous fat in most adults. The intramuscular lengths commonly supplied on luer-lock barrels are 1 to 1.5 inches. There is a second problem for anything oil-based: a 29–31G insulin needle draws viscous solution very slowly, which is why luer-lock barrels are used with a wide gauge to draw and a finer one to inject. Whether an injection should be intramuscular at all is a prescriber’s call.
What happens if I use the wrong syringe?
You get the number you were told and a volume you did not expect. Reading a millilitre instruction on an insulin barrel gives you 1% of the intended dose; reading a unit instruction on a luer-lock gives you 100 times it, if it fits at all. The FDA label for HUMULIN R U-500 puts the same warning in patient language: "You can give yourself the wrong dose if you use any other syringe, such as a U-100, tuberculin or allergy syringe", and quantifies its own version of the error — "using a U-100 syringe can give you a 5 times overdose". Check the printed scale on the barrel, not the colour of the cap, the size of the needle or which box it came out of.
Are there syringe scales other than U-100?
Three more in ordinary circulation, and all of them look like an insulin syringe. U-500 barrels exist for concentrated insulin and are marked with a green U-500 symbol and green needle shield; the HUMULIN R U-500 label states that "Each line on the U-500 syringe measures 5 units of U-500 insulin" and that using a U-100 barrel instead "can give you a 5 times overdose". U-40 is the veterinary scale, 40 units to the millilitre, so a U-100 instruction drawn on one delivers 2.5 times the volume — FDA-listed and in commercial distribution in the United States. And a luer-lock barrel is not a scale at all, it is millilitres. The insulin syringe conversion chart tabulates the U-40 error in full.
How far apart are the lines on an insulin syringe?
We could not source an answer, and this page will not repeat one it cannot stand behind. Searching manufacturer instructions-for-use, FDA device listings and 510(k) summaries across the major suppliers produced exactly one statement about sub-unit marking: embecta’s, that its insulin syringes are "Designed with large dose unit markings, with ½ unit scale marking available on the 0.3mL barrel". That is an availability claim about one barrel. The widely repeated figures — one unit on a 0.5 mL, two units on a 1 mL — trace back to patient handouts and content pages rather than to labelling. Hold your own barrel up and count the lines between two numbered marks before you set a small dose on it.
Is a 1 mL insulin syringe the same as a 1 mL syringe?
They hold the same volume and are numbered completely differently. The insulin one runs 0 to 100 in units; the luer-lock one runs 0 to 1 in millilitres. Same capacity, same length, scales a hundredfold apart. The insulin barrel also has a fixed fine short needle and the luer-lock takes whatever you screw onto it — BD lists 1 mL Luer-Lok syringes both bare and with needles from 20G × 1 inch downwards, and 3 mL ones supplied with 22G, 23G and 25G × 1 inch. If the two are in the same drawer, keep them in separate boxes and write your instructions with the unit attached.
