Why nobody can tell you what TRT costs
Three numbers get quoted for the same treatment. A telehealth subscription quotes one monthly figure that folds the drug, the prescriber and sometimes the labs together. A pharmacy quotes a price per vial, which says nothing about how long the vial lasts. A clinic quotes a price per injection, which depends entirely on how often you go. None of them is comparable to the others until you annualise all of it and divide by twelve.
There is no authoritative public price list for testosterone cypionate. Cash prices for the same vial, the same NDC, differ severalfold between two pharmacies in the same town and move with discount cards and state. So this calculator does not tell you what a vial costs — it asks you. Ring two pharmacies, get the cash price for a 10 mL vial, and put the number in. Everything downstream of that is arithmetic, and the arithmetic is where the surprises live.
The commonest error is treating a month as four weeks. Four injections a week apart is 48 a year; a year has 52. Costing the month rather than the year understates the bill by 8.3% before anything else goes wrong. The second commonest is forgetting that a vial does not divide evenly: a 10 mL 200 mg/mL vial holds 2,000 mg, which is 40 clean doses of 50 mg but 26 doses of 75 mg with 50 mg left in the glass that you still paid for.
The formula
mg per vial = vial volume (mL) × concentration (mg/mL)
mg per injection = weekly dose (mg) ÷ injections per week
doses per vial = ⌊ mg per vial ÷ mg per injection ⌋
vials per year = (injections per week × 52) ÷ doses per vial
cost per year = vials × vial price + injections × supplies
+ labs × lab price + consults × consult fee
cost per month = cost per year ÷ 12
Annualise first, divide by twelve last. A month is 4.33 weeks, not four, and the floor on doses per vial is what makes the leftover in the vial a cost rather than a rounding error.
Worked example
100 mg a week in two injections from a 10 mL 200 mg/mL vial. Suppose — and this is a supposition, not a quoted price — your pharmacy charges $90 a vial, supplies run $0.30 an injection, and you pay for 2 lab panels at $60 and 2 consultations at $75:
- mg per injection = 100 ÷ 2 = 50 mg
- doses per vial = ⌊2,000 ÷ 50⌋ = 40
- injections per year = 2 × 52 = 104
- vials per year = 104 ÷ 40 = 2.6
- testosterone = 2.6 × $90 = $234.00 a year, $19.50 a month
- everything else = syringes and supplies $31.20, blood work $120.00, consultations $150.00
- total = $535.20 a year ÷ 12 = $44.60 a month

Frequently asked questions
- How much does TRT cost per month?
- It depends far more on the route than on the dose. On the self-pay pharmacy route, 100 mg a week in two injections uses 2.6 vials of 10 mL 200 mg/mL testosterone a year — so at a vial price of $90 the drug itself is $19.50 a month, and adding two lab panels and two consultations at the prices supposed above takes the total to $44.60. Online providers in our directory list monthly plans from $77.00 to $135.00. The drug is rarely the expensive part.
- How much does TRT cost without insurance?
- Cash-pay is the default for most men on TRT, and the whole bill is the vial, the syringes, the blood work and the prescriber. The chart above runs from $40 to $140 a vial because that is the honest spread — there is no published national cash price for testosterone cypionate, and the same 10 mL vial is quoted at very different numbers by pharmacies a few miles apart. Ring two or three, ask for the cash price on the 200 mg/mL 10 mL vial by name, and put the lower one into the calculator.
- Does insurance cover TRT?
- Insurers generally treat testosterone as covered for documented hypogonadism and not covered as an optimisation or anti-ageing treatment, which is why the paperwork matters more than the price. The Depo-Testosterone label itself requires the diagnosis be confirmed with morning serum testosterone measured on at least two separate days and below the normal range before treatment starts, and that is the evidence a prior authorisation asks for. Even when the drug is covered you still pay a copay or deductible, and the monitoring labs may sit under a different benefit — so a covered prescription is a smaller bill, not no bill.
- Why is the telehealth price higher than the advertised monthly?
- Because the advertised figure is the subscription, not the bill. A $99 a month plan is $1,188 a year before anything the plan excludes — a joining or intake fee, blood work the plan does not include, and the syringes and sharps container in the self-injection routes. Add a single $150 panel the plan does not cover and the real figure is $111.50 a month, not $99. Read the listing for what the monthly includes before comparing it to anything.
- What does TRT cost at month three, once the introductory price ends?
- Frequently a good deal more than at month one. PeterMD, for instance, lists its injectable TRT at $79 a month on a twelve-month commitment or $139 a month paying monthly — the same treatment, $720 a year apart, as listed by the provider. Where a plan advertises a first-month or first-quarter rate, the number to compare is the one you will be paying in month three and every month after, multiplied by twelve. Work out the twelve-month total on the rate you settle at, not the rate you join at.
- How many vials of testosterone do I need a year?
- At 100 mg a week from a 10 mL 200 mg/mL vial, 2.6 — that vial holds 2,000 mg, which is 40 injections of 50 mg, and a year is 104 injections at twice weekly. At the same dose from a 100 mg/mL vial you need twice as many, because the vial holds half the drug. This is why the concentration on the label changes the annual bill even when the price per vial looks similar.
- Is the 200 mg/mL vial cheaper than the 100 mg/mL vial?
- Per milligram, almost always, and the gap is larger than people expect: a 10 mL 200 mg/mL vial holds 2,000 mg against 1,000 mg in the 10 mL 100 mg/mL vial, so unless the stronger vial is priced at more than double it wins. The concentration is not a dosing decision — the prescribed milligrams stay the same and the volume you draw halves — but it is the single biggest lever on the medication line of the budget, so it is worth asking which your pharmacy stocks and at what cash price.
- How often do I need blood work on TRT, and what does that add?
- The Endocrine Society guideline has clinicians reassess symptoms, testosterone and haematocrit three to six months after starting and annually after that, so a realistic year is two to three panels in the first year and one or two thereafter. The panels are a real line item and are handled separately by every route: some telehealth subscriptions bundle them, some bill them, and self-pay carries them entirely. Put the number of panels and the price you actually pay into the calculator rather than assuming they are included.
