Start at the first grid. Find the milligrams printed on your vial down the left, the millilitres of bacteriostatic water you added across the top, and read off the large figure: that is how many micrograms one unit mark on a U-100 insulin syringe carries. The small figure underneath is the concentration in mg/mL, which is the same thing expressed the way a label would write it. Concentration is nothing more than the vial's contents divided by the water added — a 5 mg vial in 2 mL is 2.5 mg/mL, and at 2.5 mg/mL one mark is 25 mcg. Once you have that one number you never need the vial again; every dose is one division.

The second grid does the division for you. Find your dose in micrograms down the left and your micrograms-per-mark across the top, and it gives the number of units. Where a second microgram figure appears underneath, the dose does not fall on a mark and that is what the reachable quarter-unit actually delivers — 3.25 units at 75 mcg a mark is 243.75 mcg, not the 250 you asked for. The third grid runs the whole thing backwards: pick the micrograms-per-mark you want and it tells you how much water to add to get it.

The mistake this sheet exists to stop is not the syringe conversion. It is the factor of 1000. A vial is labelled in milligrams and a peptide dose is quoted in micrograms, so a 250 mcg dose is 0.25 mg — and there is not enough drug in a 5 mg vial for the mistake to announce itself. Every figure on this page is in the unit its column header says it is in, and the row headers carry both so you can check yourself against them.

This chart converts numbers. It does not choose them. There is no dose, no frequency and no cycle on this page for any compound, and that is not caution for its own sake — for most of what people reconstitute at home there is no sourceable figure to print. No approved label, no dose-ranging trial, no regulator-reviewed instruction. A number invented to fill the gap would look exactly like a real one, and on a sheet taped to a cupboard it would outlive every caveat around it. Bring your own dose; this does the arithmetic on it.

Nothing in this grid depends on which peptide is in the vial — it is mass divided by volume. The lyophilised powder's own displaced volume is ignored, as every manufacturer's reconstitution table does; for a few milligrams of solid it sits below what the syringe reading the result can resolve. Where a product arrived with a printed reconstitution table, that table wins: an FDA label accounts for deliberate overfill and this grid uses the nominal mass. EGRIFTA WR is the worked case — an 11.6 mg vial in 1.3 mL divides out to 8.92 mg/mL, and the label states 8 mg/mL.

Two kinds of cell are greyed, for two different reasons. Anything over 100 units is more than 1 mL and no insulin barrel holds it — that is the instrument, not a judgement. The rest are greyed where the quarter-unit rounding moves the delivered dose by more than 10%, which is our threshold and not a figure from any label: the arithmetic is still correct, the concentration is simply the wrong one for that dose. In both cases the fix is the same — reconstitute to a different strength so the dose lands on more marks. A quarter unit is also not a printed line on any barrel we could source; it is the finest split anyone can reasonably eyeball between two lines.

Working backwards — water to add for a chosen strength

The inverse of the first grid, and the more useful direction if the vial is still dry. Choose what you want one mark to be worth, read across from your vial, and that is the bacteriostatic water to add.

Diluting further does not make more drug — it makes the same drug easier to measure. Greyed at the top end are volumes larger than the 30 mL multiple-dose vial bacteriostatic water is normally supplied in, which means two bottles. Greyed at the bottom end are volumes under 0.2 mL, where the water itself is fewer than 20 marks on the barrel you would measure it with; that cut is our judgement about measuring, not a labelled limit. Hospira's label gives a separate ceiling on the water for a different reason — animal studies of preparations containing 0.9% benzyl alcohol "indicated that an estimated intravenous dose up to 30 mL may be safely given to an adult without toxic effects" — and that figure is per dose, not per vial.

This is the single most common misunderstanding about reconstitution, so it gets its own table: adding more water does not stretch the vial. A 5 mg vial holds twenty 250 mcg doses whether you mixed it with 1 mL or 5 mL — all that changes is whether the draw is 5 units or 25. Part doses are not counted. How long that many doses lasts depends on a frequency this page does not supply.

Beyond-use dates: which label actually says what

Reconstitution sheets print “28 days” next to the bacteriostatic water as though it came off the bottle. It does not. This is where each figure genuinely comes from.

We pulled all seven standalone Bacteriostatic Water for Injection, USP labels on DailyMed — Hospira, Fujian Genohope, ProPharma, Henry Schein, HealthFirst, Medical Purchasing Solutions, US MedSource — and searched every one. None contains the string “28”, “day”, “puncture”, “discard” or “expir”. What the water label does say is that it is supplied “in a multiple-dose container from which repeated withdrawals may be made” and “Do not store reconstituted solutions of drugs for injection unless otherwise directed by the manufacturer of the solute” — it hands the question to the drug. The 28 days is real and comes from two other places: CDC, citing USP General Chapter 797, that “Once a multi-dose vial is opened (e.g., needle-punctured) the vial should be dated and discarded within 28 days unless the manufacturer states another date for that opened vial”; and from labels that supply the water, such as EGRIFTA WR, which tells patients to “Throw away (discard) any Bacteriostatic Water for Injection left in the bottle 28 days after first use.” The mixed-drug column is the point of the table — three FDA labels, three different answers, all set by the drug and none by the water.

Frequently asked questions

How many units is 250 mcg of peptide?
It depends entirely on what you mixed, which is why this page is a grid rather than a number. At 25 mcg a mark — a 5 mg vial in 2 mL — 250 mcg is 10 units. At 50 mcg a mark (5 mg in 1 mL) it is 5 units, and at 10 mcg a mark (5 mg in 5 mL) it is 25 units. Same dose, same vial, three different marks — the water volume is the variable.
How much bacteriostatic water should I add to a 5 mg vial?
Whatever makes your dose land on a mark you can read. 5 mg in 1 mL gives 50 mcg a unit, in 2 mL gives 25, in 2.5 mL gives 20 and in 5 mL gives 10. None of them changes how much peptide you own — a 5 mg vial is 5000 mcg in every case. The third grid on this page is that decision laid out in reverse: pick the micrograms-per-mark you want and it gives the millilitres.
How many mcg are in a mg?
1000, exactly. A 2 mg vial is 2000 mcg, a 5 mg vial is 5000 mcg, a 10 mg vial is 10000 mcg. This is the conversion worth being paranoid about: a 250 mcg dose is 0.25 mg, so anyone who reads it as 250 mg is attempting fifty times the entire contents of a 5 mg vial. Work in one unit the whole way through and convert once.
How much of this peptide should I inject?
This chart will not answer that, and the reason is not squeamishness — for most of these compounds there is no sourceable answer to print. No approved label, no dose-ranging study, no regulator-reviewed instruction. FDA placed BPC-157, ipamorelin acetate, kisspeptin-10 and others in category 2 of its compounding bulks review, and wrote of BPC-157 that "FDA has identified no, or only limited, safety-related information for the proposed routes of administration. Therefore, the agency lacks sufficient information to know whether the drug would cause harm when administered to humans." When the regulator says it does not know, a directory printing a milligram figure would be making one up. Ask a clinician who can see your history.
Does bacteriostatic water expire 28 days after you puncture it?
That figure is real, but it does not come from the bacteriostatic water label — we checked all seven Bacteriostatic Water for Injection, USP labels published on DailyMed and none of them states a post-puncture period at all. It comes from CDC, citing USP General Chapter 797: "Once a multi-dose vial is opened (e.g., needle-punctured) the vial should be dated and discarded within 28 days unless the manufacturer states another date for that opened vial." It also appears on drug labels that supply the water as their diluent — EGRIFTA WR instructs patients to "Throw away (discard) any Bacteriostatic Water for Injection left in the bottle 28 days after first use." Date the bottle at first puncture; just do not tell yourself the bottle told you to.
How long does the peptide keep once it is mixed?
That belongs to the compound, not to the water, and the three FDA labels that use bacteriostatic water as a diluent give three different answers: EGRIFTA WR is discarded 7 days after mixing, SEROSTIM 4 mg may be refrigerated for up to 14 days, and NOVAREL "should be refrigerated and used within 30 days". The water label itself defers — "Do not store reconstituted solutions of drugs for injection unless otherwise directed by the manufacturer of the solute". For an unapproved peptide there is no manufacturer of the solute in that sense and no figure has been established, so this page does not print one.
Does adding more water make the vial last longer?
No, and the fourth table on this page exists to settle it. A 5 mg vial contains 20 doses of 250 mcg whether you reconstitute it with 1 mL or 5 mL, because reconstitution does not create peptide. What more water buys is a bigger draw: the same 250 mcg is 5 units at 1 mL and 25 units at 5 mL, and 25 marks are far harder to misread than 5.
What do I do when my dose does not land on a syringe mark?
Draw the nearest quarter unit and know what it delivers, or change the concentration. A 250 mcg dose from a 15 mg vial in 2 mL — 75 mcg a mark — is 3.333 units in theory, 3.25 units on the barrel and 243.75 mcg in the syringe, about 2.5% short. Reconstituting the same vial with 3 mL instead gives 50 mcg a mark and puts the dose on exactly 5 units. Choosing a water volume that makes your dose a whole number of units is the fix, and it is free.
Why does my vial say a different concentration from this chart?
Because a manufacturer accounts for overfill and this chart uses the nominal milligrams you read off the label. EGRIFTA WR is the clearest example: an 11.6 mg vial reconstituted with 1.3 mL divides out to 8.92 mg/mL, but the label states "Reconstitute 1 vial of EGRIFTA WR lyophilized powder with 1.3 mL of diluent (8 mg per mL)." The vial is deliberately overfilled so the stated strength is reliably deliverable. If your product came with a printed reconstitution table, use it. This chart is for the vial that arrived with nothing but a milligram figure on it.
Which syringe should I use to draw a peptide dose?
A U-100 insulin barrel, and the smallest one your draw fits in, because the marks are further apart on a smaller barrel. U-100 means 100 units to the millilitre, so one mark is 0.01 mL and a 0.3 mL barrel is numbered to 30, a 0.5 mL to 50 and a 1 mL to 100. Anything over 100 units is more than a millilitre and no insulin syringe holds it — reconstitute to a stronger concentration rather than splitting the injection. Check the barrel is marked U-100 and not U-40; the veterinary scale delivers 2.5 times the volume at the same number.
