Semaglutide Dosage Chart

Find your product across the top, not your milligrams down the side. The same number means different things in different columns: 2 mg is the ceiling for Ozempic, a mid-titration step for Wegovy tablets and not a dose at all for Rybelsus. Read down the column you were actually prescribed.

Wegovy prints real week numbers in its label, so the Wegovy column is the label’s own calendar. Ozempic and Rybelsus instead state a minimum interval before each optional increase, so their later rows are the earliest week or day the step is permitted — greyed to mark that the label makes them conditional on needing more effect, not automatic.

Two figures are deliberately absent. There is no FDA-labelled schedule for compounded semaglutide, so no compounded ladder appears here; if you have a vial of powder, the dose is whatever your prescriber wrote and the units converter turns it into syringe marks. And there is no milligram equivalence between the injection and the tablets — the label states the two tablet products “are not substitutable on a mg-to-mg basis” — so only the specific switches the labels name are listed below.

Injectable semaglutide, week by week

Once-weekly subcutaneous injection. Both products start at the same dose and part company at week 13.

Greyed figures are steps the label makes conditional on additional glycaemic control being needed, shown at the earliest week permitted. Ozempic names 0.5 mg, 1 mg and 2 mg all as maintenance dosages, so staying at 0.5 mg indefinitely is on-label.

Oral semaglutide, day by day

One tablet daily on an empty stomach with up to 4 ounces of water, then nothing for 30 minutes. Three products, three different milligram ladders.

The first 30 days are initiation only — for Rybelsus and Ozempic tablets the label states that starting dosage “is not effective for glycemic control”. Greyed figures are conditional increases, shown at the earliest day permitted.

Where the three diverge

The same molecule under three brand names. Every row is a point where carrying a figure across columns gives the wrong answer.

Milligrams do not carry across columns. The oral titration totals hundreds of milligrams against the injection’s single figures because oral semaglutide is absorbed far less efficiently, not because the dose is larger. The Wegovy label carries no renal or hepatic dosage-adjustment section at all. What it does say, in section 12.3, is that “No clinically significant differences in semaglutide pharmacokinetics were observed based on … renal impairment [estimated glomerular filtration rate (eGFR) ≥30 to <90 mL/minute], or mild, moderate, and severe hepatic impairment (Child-Pugh Class A-C)”. That is a pharmacokinetic finding, not a dosing instruction, so no dosing instruction is stated here.

The only switches the labels state

No general conversion exists between these products. These pairs are the whole of what the FDA labels authorise.

Anything not in this table has no labelled equivalence, including any milligram conversion between an injection and a tablet outside the pairs above.

What each presentation contains

From section 3 of each label. The concentration column is the label’s own figure, not ours.

Never back-compute a concentration from the carton. Ozempic’s 2 mg/3 mL pen is labelled 0.68 mg/mL, and 2 divided by 3 is 0.67 — the pens are overfilled so the stated number of doses can be withdrawn. Wegovy’s 9.6 mg/3 mL FlexTouch is the one multi-dose device where the arithmetic matches.

Frequently asked questions

What is the semaglutide dosage schedule?

There is no single schedule — there are five approved ones. Ozempic injection starts at 0.25 mg once weekly for 4 weeks, then 0.5 mg, with optional increases to 1 mg and 2 mg after at least 4 weeks each. Wegovy injection runs 0.25, 0.5, 1 and 1.7 mg for 4 weeks apiece and reaches its 2.4 mg maintenance dose at week 17. Rybelsus runs 3 mg for 30 days, 7 mg from day 31 and 14 mg from day 61 if more glycaemic control is needed.

Is the Wegovy dose the same as the Ozempic dose?

Only for the first 8 weeks. Both start at 0.25 mg once weekly and both step to 0.5 mg at week 5, after which they separate: Ozempic’s maximum is 2 mg once weekly, while Wegovy continues to 1.7 mg at week 13, 2.4 mg at week 17 and, for weight reduction in adults who have tolerated 2.4 mg for at least 4 weeks, up to 7.2 mg once weekly. They are the same molecule at different ceilings for different indications.

How long does it take to reach 2.4 mg of semaglutide?

On the Wegovy injection schedule, 17 weeks — four 4-week steps at 0.25, 0.5, 1 and 1.7 mg, with 2.4 mg from week 17 onward. That titration uses 7 mg of drug before the first maintenance-range dose. The label adds that if a dose is not tolerated, escalation can be delayed by 4 weeks, and that 2.4 mg is not a dose Ozempic ever reaches.

What is the maximum dose of semaglutide?

It depends entirely on the product. Ozempic injection: 2 mg once weekly. Wegovy injection: 7.2 mg once weekly, and only for weight reduction in adults. Rybelsus: 14 mg once daily. Ozempic tablets: 9 mg once daily. Wegovy tablets: 25 mg once daily. Five products, five different ceilings.

Why are Rybelsus and Ozempic tablets different milligrams for the same thing?

Both are oral semaglutide for type 2 diabetes, but they are formulated differently and the label states plainly that they "are not substitutable on a mg-to-mg basis". The label pairs them only at specific doses when switching: Rybelsus 7 mg corresponds to Ozempic tablets 4 mg, and Rybelsus 14 mg to Ozempic tablets 9 mg. Outside those two pairs there is no equivalence to apply.

Is there a dosage chart for compounded semaglutide?

Not one that can be sourced, and this page does not invent one. Compounded semaglutide is not an FDA-approved product and carries no approved escalation schedule, so any ladder published for it is someone’s house protocol rather than a label. What can be stated is the arithmetic: a compounded vial’s concentration depends on how much bacteriostatic water was added, so 0.25 mg is 12.5 units at 2 mg/mL and 2.5 units at 10 mg/mL. Convert the dose your prescriber wrote; do not copy a branded schedule onto a compounded vial.

What happens if I miss a dose?

The rules differ by product. Ozempic injection: administer within 5 days of the missed dose, otherwise skip it. Wegovy injection: if the next scheduled dose is more than 2 days away take it as soon as possible, otherwise skip it — and if 2 or more consecutive doses are missed, the label says to reinitiate dosage escalation at a lower dosage. All three tablet products: skip the missed dose and take the next one the following day.
