Brand — two products, one name
Ozempic
One brand name covers two different medicines: a once-weekly injection that stops at 2 mg, and a once-daily tablet that goes to 9 mg and shares its label with Rybelsus.

Why "Ozempic" does not identify a dose
Semaglutide is sold in the United States under three brand names across five distinct products. Ozempic accounts for two of them. The weekly injection is approved for type 2 diabetes, cardiovascular risk reduction and chronic kidney disease, and its label caps the dose at 2 mg once weekly. The tablet is approved for type 2 diabetes and cardiovascular risk reduction, is taken every day rather than every week, and its maintenance dose runs to 9 mg.
That makes a sentence like "I am on 9 mg of semaglutide" unresolvable without the box. Nine milligrams is the top maintenance dose of an Ozempic tablet, and it is also the third rung of a four-rung escalation on a Wegovy tablet, where the maintenance dose is 25 mg daily. The first three rungs of those two ladders — 1.5 mg, 4 mg, 9 mg — are numerically identical, and then one stops and the other keeps going.
None of this is a property of semaglutide. The molecule has no maximum dose, no indication and no device; each of those belongs to a product. That is the entire reason this page is separate from the compound reference, and it is what the table below is for.
What is true of Ozempic and not of semaglutide
Each row is something a reader would reasonably assume belongs to the molecule, and does not. These are the facts that change when the brand name on the carton changes, while the drug inside it does not.
| Attribute | Ozempic | Elsewhere, same molecule |
|---|---|---|
| Maximum labelled doseSemaglutide has no maximum dose. Five approved products carry five different ceilings — 2, 7.2, 9, 14 and 25 mg — and the highest weekly injectable one is 3.6× the Ozempic ceiling. A dose quoted without a brand cannot be checked against anything. | Ozempic injection: 2 mg once weekly. Ozempic tablets: 9 mg once daily. | Wegovy injection: up to 7.2 mg once weekly for weight reduction in adults. Wegovy tablets: 25 mg once daily. Rybelsus: 14 mg once daily. |
| What it is approved to treatWeight loss is not a labelled indication of Ozempic. Prescribing it for weight is off-label use of this brand, even though the identical molecule is on-label for weight under another one. | Type 2 diabetes, cardiovascular risk reduction, and — injection only — chronic kidney disease in type 2 diabetes. | Wegovy is the semaglutide brand indicated for weight reduction, for cardiovascular risk reduction in obesity or overweight, and for noncirrhotic MASH with F2–F3 fibrosis under accelerated approval. |
| Dosing intervalThe interval is a property of the product, not the brand and not the molecule. "Take your Ozempic" is an incomplete instruction. | Weekly for the injection; daily, fasting, 30 minutes before anything else for the tablet. | Rybelsus and Wegovy tablets are daily; Wegovy injection is weekly. The same brand name covers both intervals. |
| The first three rungs of the oral ladderThree identical numbers that mean opposite things. Someone told to "stay on 9 mg" and someone told to "move up from 9 mg" are both following their own label correctly. | Ozempic tablets: 1.5 mg → 4 mg → 9 mg, where 9 mg is the maintenance dose. | Wegovy tablets: 1.5 mg → 4 mg → 9 mg → 25 mg, where 9 mg is a 30-day escalation rung on the way to maintenance. |
| Concentration in the deviceThere is no "mg per unit" for Ozempic, because the pen dial reads in milligrams and only offers fixed increments. A conversion to insulin-syringe units, which is the commonest thing people search for, does not apply to any Ozempic presentation. | Four presentations under one brand: pens labelled 0.68, 1.34 and 2.68 mg/mL, and a single-dose prefilled syringe for which section 3 of the label prints no mg/mL at all. Section 16 adds 0.5, 1 and 2 mg/mL for the syringes; the two sections describe the same product at different levels of detail, and this page follows section 3. | Compounded and vialled semaglutide is sold at concentrations set by the compounder, with no labelled equivalent. |
| Pediatric useOne molecule, one age group, two opposite labelled positions — decided entirely by which brand is on the carton. | Safety and efficacy of OZEMPIC have not been established in pediatric patients. | Wegovy injection is indicated to reduce excess body weight in pediatric patients aged 12 years and older with obesity. |
| Storage after first useTwo Ozempic presentations in the same fridge have different discard dates. The 56-day figure belongs to the pen and does not transfer to the syringe. | Pen: 56 days at 15–30 °C or refrigerated. Prefilled syringe: 28 days at 8–30 °C. | Tablets have no cold chain at all, and compounded product carries a pharmacy beyond-use date rather than a labelled limit. |
Boxed warning — risk of thyroid C-cell tumors
In rodents, semaglutide causes dose-dependent and treatment-duration-dependent thyroid C-cell tumors at clinically relevant exposures. It is unknown whether OZEMPIC causes thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in humans as human relevance of semaglutide-induced rodent thyroid C-cell tumors has not been determined.

What this product is approved to treat
- —as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
- —to reduce the risk of major adverse cardiovascular events (cardiovascular death, non-fatal myocardial infarction or non-fatal stroke) in adults with type 2 diabetes mellitus and established cardiovascular disease.
- —to reduce the risk of sustained eGFR decline, end-stage kidney disease, and cardiovascular death in adults with type 2 diabetes mellitus and chronic kidney disease.
The labelled ladder
| When | Dose | What the label attaches to this step |
|---|---|---|
| Weeks 1–4 | 0.25 mg once weekly | — |
| Weeks 5–8 | 0.5 mg once weekly | — |
| Weeks 9–12 | 1 mg once weekly | Conditional — if additional glycemic control is needed. The label states a minimum interval rather than a date, so weeks 9–12 is the earliest this step can begin, not a scheduled one. Also the maintenance dosage in type 2 diabetes with chronic kidney disease. |
| Week 13 onward | 2 mg once weekly | Conditional — if additional glycemic control is needed. The label states a minimum interval rather than a date, so week 13 onward is the earliest this step can begin, not a scheduled one. |
Ceiling. "The maximum recommended dosage is 2 mg once weekly." Nothing above 2 mg weekly is labelled under this brand. A weekly semaglutide injection of 7.2 mg exists, but only as Wegovy and only for weight reduction in adults.

Administration, as the label words it
- —Administer OZEMPIC once weekly, on the same day each week, at any time of the day, with or without meals.
- —The day of weekly administration can be changed if necessary as long as the time between two doses is at least 2 days (>48 hours).
- —If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day.
- —Always use the dose counter and the dose pointer to see how many mg you select. Do not set the dose by counting the number of clicks you hear.

This product has no label of its own. It is described inside the Rybelsus prescribing information, under a joint title covering both brands. Everything quoted for Ozempic tablets below is taken from that shared document, which is why a search for a “Ozempic tablets label” returns Rybelsus.
What this product is approved to treat
- —as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
- —to reduce the risk of major adverse cardiovascular (CV) events (CV death, non-fatal myocardial infarction or non-fatal stroke) in adults with type 2 diabetes mellitus who are at high risk for these events.
The labelled ladder
| When | Dose | What the label attaches to this step |
|---|---|---|
| Days 1–30 | 1.5 mg once daily | Initiation only. The label states this dosage is not effective for glycemic control. |
| Days 31–60 | 4 mg once daily | — |
| Day 61 onward | 9 mg once daily | Conditional — if additional glycemic control is needed. The label states a minimum interval rather than a date, so day 61 onward is the earliest this step can begin, not a scheduled one. |
Ceiling. The label sets no higher rung than 9 mg orally once daily for Ozempic tablets. It is written as the escalation target, not as a "maximum recommended dosage" — the injection label uses that phrase, this one does not.
How it is supplied
| Presentation | Total strength | Concentration | Delivers |

This statement belongs to Ozempic tablets. It is not a statement about semaglutide, and another brand of the same molecule may say the opposite.
The same molecule under other brand names
Everything below is semaglutide. None of it is Ozempic, and a dose from one column carried into another is not a smaller or larger version of the same instruction — it is a different product’s schedule.
| Brand | Form | Approved for | Ladder | Maximum |
|---|---|---|---|---|
| Wegovy injection | Subcutaneous, once weekly | Weight reduction; cardiovascular risk reduction in established CV disease with obesity or overweight; noncirrhotic MASH with F2–F3 fibrosis (accelerated approval) | 0.25 mg → 0.5 mg → 1 mg → 1.7 mg → 2.4 mg | 7.2 mg once weekly — weight reduction in adults only, after tolerating 2.4 mg for at least 4 weeks |
| Wegovy tablets | Oral, once daily | Weight reduction; cardiovascular risk reduction in obesity or overweight | 1.5 mg → 4 mg → 9 mg → 25 mg | 25 mg once daily |
| Rybelsus tablets | Oral, once daily | Type 2 diabetes — glycaemic control, and cardiovascular risk reduction in type 2 diabetes at high risk | 3 mg → 7 mg → 14 mg | 14 mg once daily |
Switching between products
The two oral semaglutides are not milligram-equivalent
RYBELSUS and OZEMPIC tablets are not substitutable on a mg-to-mg basis.
Ozempic tablets and Rybelsus
Switching between them is only allowed after the first 30 days
Do not switch between RYBELSUS and OZEMPIC tablets during the initiation phase (Days 1 to 30).
Ozempic tablets and Rybelsus
Only one injection dose has a labelled oral switch
Patients taking the 0.5 mg dose of OZEMPIC injection may switch to OZEMPIC tablets. One week after discontinuing 0.5 mg of subcutaneous OZEMPIC injection, start 4 mg or 9 mg of OZEMPIC tablets orally once daily.
Ozempic injection to Ozempic tablets
These are the only switches the labels name. There is no general milligram equivalence between one product and another, and this page does not supply one.
| From | To | When |
|---|---|---|
| Ozempic injection, 0.5 mg once weekly | Rybelsus tablets, 7 mg or 14 mg once daily | One week after discontinuing the injection |
| Ozempic injection, 0.5 mg once weekly | Ozempic tablets, 4 mg or 9 mg once daily | One week after discontinuing the injection |
| Rybelsus tablets, 7 mg once daily | Ozempic tablets, 4 mg once daily | The next day, and not during days 1–30 |
| Rybelsus tablets, 14 mg once daily | Ozempic tablets, 9 mg once daily | The next day, and not during days 1–30 |
What this page does not tell you
- Price — list, cash or copay
- No price appears anywhere on this page, and no price is emitted in its structured data. Ozempic pricing varies by presentation, pharmacy, insurance design and manufacturer programme, and we could not find a source for a current US figure that we could cite and re-verify the way every other number here is cited. A stale or wrong price is the most consequential thing a page like this could print, so the field is absent rather than estimated.
- How many insulin-syringe units a dose equals
- This is the most-searched question about Ozempic and it has no answer for this brand. Every Ozempic presentation is a fixed-dose device — a dial-a-dose pen reading in milligrams, or a single-dose prefilled syringe. The label instructs using the dose counter, not counting clicks, and there is no vial to draw from. Any "Ozempic units chart" is describing a compounded product, not this one.
- Expected weight loss
- Ozempic is not indicated for weight reduction, so its label reports no weight-loss endpoint to quote. Figures circulating for "semaglutide weight loss" come from the Wegovy programme at doses Ozempic does not reach.
- Tablet storage and pediatric detail
- The oral label covers Rybelsus and Ozempic tablets jointly and does not split those sections by brand. Rather than attribute a shared statement to one brand, this page links the SPL and leaves the field blank.
Frequently asked questions
- Is Ozempic an injection or a tablet?
- Both, under the same brand name. Ozempic injection is a once-weekly subcutaneous product with its own FDA label (NDA209637) running 0.25 mg to a maximum of 2 mg weekly. Ozempic tablets are a once-daily oral product described inside the Rybelsus label, running 1.5 mg to 9 mg daily. They are different medicines with different ladders, and a prescription for one is not a prescription for the other.
- What is the maximum dose of Ozempic?
- For the injection, the label states: "The maximum recommended dosage is 2 mg once weekly." For the tablets, the highest rung the label describes is 9 mg orally once daily. Neither figure applies to semaglutide generally — Wegovy injection goes to 7.2 mg once weekly and Wegovy tablets to 25 mg once daily, so quoting a semaglutide dose without naming the brand does not identify a treatment.
- Is Ozempic approved for weight loss?
- No. The Ozempic label lists three indications: glycemic control in adults with type 2 diabetes, reduction of major adverse cardiovascular events in type 2 diabetes with established cardiovascular disease, and reduction of sustained eGFR decline, end-stage kidney disease and cardiovascular death in type 2 diabetes with chronic kidney disease. Weight reduction is a labelled indication of Wegovy, which is the same molecule under a different brand at a different ceiling.
- How many units is a 0.5 mg Ozempic dose?
- The question does not apply to any Ozempic presentation. The pens are dial-a-dose devices whose counter reads in milligrams and which only offer fixed increments — the 2 mg/3 mL pen gives 0.25 mg or 0.5 mg and nothing between. The Instructions for Use state: "Always use the dose counter and the dose pointer to see how many mg you select," and separately, "Do not set the dose by counting the number of clicks you hear." The single-dose prefilled syringes are pre-filled at 0.25, 0.5 or 1 mg per 0.5 mL and are not dialled at all.
- Why does the Ozempic pen come in three different concentrations?
- Because each pen is built around the doses it gives. The 2 mg/3 mL pen is labelled 0.68 mg/mL and covers 0.25 mg and 0.5 mg; the 4 mg/3 mL pen is 1.34 mg/mL for 1 mg; the 8 mg/3 mL pen is 2.68 mg/mL for 2 mg. Note that none of those three is total strength divided by total volume — 2 ÷ 3 is 0.667, not 0.68 — because the containers are overfilled, so a concentration worked out from the carton comes out wrong. The single-dose prefilled syringe is a fourth presentation for which section 3 of the label prints no concentration.
- Can I swap Ozempic tablets for Rybelsus at the same milligrams?
- No. The shared label states plainly: "RYBELSUS and OZEMPIC tablets are not substitutable on a mg-to-mg basis." It also directs that patients should not switch between them during the initiation phase, days 1 to 30. After that, the label pairs 7 mg of Rybelsus with 4 mg of Ozempic tablets and 14 mg of Rybelsus with 9 mg of Ozempic tablets.
- How long can an Ozempic pen stay out of the fridge?
- After first use, the pen can be stored for 56 days at 15 °C to 30 °C (59 °F to 86 °F) or in a refrigerator at 2 °C to 8 °C. That figure belongs to the pen only. The single-dose prefilled syringe is a different rule: refrigerated until its expiration date, and if needed it can be kept from 8 °C to 30 °C for up to 28 days. Before first use, an unopened pen stays refrigerated in its carton until the expiration date.
- What happens if I miss a weekly Ozempic injection?
- The label states: "If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day." The day of the week can also be changed, provided at least 2 days (more than 48 hours) separate two doses. The oral product has a different rule: skip the missed dose and take the next one the following day.
