Zip 33169 is neither. 15600 NW 15th Avenue sits in Sunshine State Industrial Park, on the Miami Gardens / Golden Glades side of the county line, inside a ~201,000-square-foot warehouse still listed as Golden Glades Fulfillment Center: concrete block from 1967, a 2004 renovation, 25-foot clear height, dock-high doors, a fenced truck court, a glass storefront glued onto industrial skin. DaVita Golden Glades Dialysis is suite D. Some cards still put dialysis in B. Suite C is the door this page is about — a styled consultation room with a round table, black chairs, a wagon-wheel chandelier, and a wall of frames — attached to a suite that also prints as The Pharmacy Hub, a B2B compounding and mail-order fulfillment house. That is not a Brickell waiting room. It is also not a guarantee you will sit in that chair.

    Hendrx Health is Dr. Tyler Hendricks's cash testosterone membership: one board-certified family-medicine MD, a $350 first visit, $395 a month, Labcorp for the blood, testosterone cypionate shipped to you, follow-ups on a screen.

    The door is suite C. Miami Gardens, FL 33169 — listings file the city as Miami; his own pages print Miami Gardens. Same zip. Phone (786) 972-7753. You can call or text. Site hendrxhealth.com. Email care@hendrxhealth.com. Instagram is @hendrxhealth. You book the first slot at hendrxhealth.com/appointments. Pay the $350 by 10:00 a.m. ET the day of the visit or they cancel it. That fee is non-refundable. It is supposed to cover the labs and the lab-review visit. Rescheduling is a $50 rebooking fee. The booking page sells Zoom. The FAQ still says initial consults happen in person in C, and follow-ups are telehealth. Ask which version of the visit you bought before you sit in Golden Glades traffic.

    Hours are not printed on their own pages, and that is why people still drive to a locked industrial park. His own pages do not print Monday. They do not print Tuesday. They do not print a weekday grid at all. Contact copy says call or text anytime at (786) 972-7753. That is a phone line, not a clock. Google currently only shows Thursday, 10 AM to 2 PM, and the hours widget currently says Closed, opens Thursday at 10:00 AM. That is a scrape, not a schedule. Third-party cards invent Monday through Friday, 10 to 2, weekend closed. Doctor listings invent 8 to 5 on weekdays. The compounding warehouse that still prints this same suite runs a different clock entirely, and dialysis next door opens at 5:30 in the morning. Do not import any of that onto C. If you only have a lunch hour, get a human on (786) 972-7753.

    The office on NW 15th

    15600 is a warehouse with a front. Sunshine State Industrial Park, I-1 zoning, land use that still reads warehouse or storage. You are west of I-95, under the Golden Glades interchange where the Palmetto (826), the Turnpike, I-95, and SR 9 pile on top of each other, a few blocks south of NW 167th. Carol City and Opa-locka sit west. North Miami is east of the interstate, a different mental map. Hard Rock Stadium and Miami Gardens Drive are north. Brickell is a different afternoon. Put 40 SW 13th in the GPS and you will miss this. Put 9440 Santa Monica Boulevard in the GPS and you will be in Beverly Hills, looking at a dissolved professional corporation. Put 6195 Lusk Boulevard in the GPS and you will be in San Diego, looking at a doctor listing that is not this membership.

    Parking is the industrial lot, not a medical garage and not a valet. The park advertises ample parking for trucks and employees. The truck court is fenced. Dock-high bays eat the pavement. DaVita patients start filling that lot at 5:30 a.m. Confirm C. If the glass says dialysis, or Hub, or a dock door, you are in the wrong visit. From I-95 you peel off at Golden Glades and work west, then south onto NW 15th. From the Palmetto you are already in the interchange; do not keep going to Doral. From the Beach it is 195 to 95 north to Golden Glades, which is not a lunch-hour trip when the stack is stacked. Miami International is the close airport. Fort Lauderdale-Hollywood is the long one. Opa-locka Executive is the one you can see from this zip.

    Dr. Tyler Hendricks is who you are seeing. Board-certified family medicine. Windsor University School of Medicine. Residency at Florida State / Lee Memorial. He puts his own name on the membership. He writes that he has been a patient of bioidentical hormones, not only a prescriber. Patients call him Tyler, Dr. Tyler, Dr. Hendricks, and, in at least one Google note, Dr Hendrix. Same man. There is no rotating roster on the site. There is no named NP, no named PA, no named medical assistant. If you need a different license in the room, this is the wrong suite.

    He does not publish a language. Miami Gardens will hand you Spanish without a banner. His pages do not. If you want the visit in Spanish, say so when you book. Do not assume the Zoom will be.

    The room they photograph is real enough to describe. Glass doors. Light wood-look floor. A round table and a long conference table. Black chairs. A credential wall. Anatomical prints. A periodic table. Waiting chairs and cognac poufs on a grey rug. Drop ceiling. A bladeless tower fan. That is a consult suite inside a fulfillment box, not an exam mill and not a med spa. The live question is whether your confirmation is for that table or for a passcode on Zoom.

    They will still try to see you when you are not in C. The three steps they publish are all a screen: tele-visit, labs at your leisure through a lab partner, tele-visit again for the first prescription. Medication is shipped. Monthly check-ins are telemedicine, they say, and then the welcome packet says video every 2 to 3 months. Members get a direct number, (786) 972-7720, to call or text him. That is not the public support line. Ask which version of the visit you are buying: the chair in C, or a screen.

    What he actually does

    Start with the screen, because that is how they sell the first hour, even when the FAQ still talks about an in-person consult in Miami Gardens.

    You book a telehealth slot with Dr. Hendricks. Labs are not ordered before that visit. On the call he reviews intake, history, and goals, then orders blood. You print the order he emails and take it to Labcorp. You can view results in the Labcorp portal. You can draw labs on your own; they say that will not reduce the membership. At the lab-review visit he walks the numbers and, if he thinks you are a candidate, writes the plan. That sequence is the office: you do not point at a board and leave with a vial. You talk, they draw, a protocol shows up. Ask whether any of those three steps happens at the round table in C.

    The product is a testosterone membership. $395 a month, all-inclusive, they say. What they print inside that number: provider consults and telehealth follow-ups, a plan with monitoring, testosterone cypionate shipped to you, an estrogen inhibitor if he thinks you need one, syringes and needles and alcohol wipes, routine labs. Membership pricing can change; they say they will give three months' notice. Leftover company copy and an old FAQ still say $295. The live number on the site is $395. Confirm it.

    The labs they name: free and total testosterone, estradiol, CBC, comprehensive metabolic panel, vitamin B12, vitamin D, thyroid panel (TSH, Free T3, T4, TPO), PSA, lipids, LH, FSH. Thyroid is a panel they draw. It is not a thyroid clinic they published. Do not drive here for a standalone thyroid practice, for GLP-1 pens, for a drip menu, for aesthetics, for B12 shots as a product, or for a premature-ejaculation protocol. Those were leftover catalog lines, not what he sells.

    How you take it: self-administered subcutaneous or intramuscular injections. Subcutaneous into belly fat is the method they now call preferred. Intramuscular they point at the glute. They will walk you through the first shot. Creams and gels exist in the FAQ as a hedge for people who only need a low dose and hate needles. Injections are what they say they can control. Do not assume a pellet, a patch, or a brand of gel unless he writes one.

    Shipping is the other half of the product. Fast shipping, they print. Store the vial at room temperature, 59 to 85°F. Cold makes cypionate crystallize. They will tell you how to warm it back. You throw the needles in a sharps box. This is mail-order testosterone with a named MD on the other end of Zoom, not a lounge with a nurse in the next chair.

    Follow-up is a mess on their own pages, which is useful. FAQ: monthly tele-check-ins, labs every 3 to 6 months. Welcome: video every 2 to 3 months. Members: text him. Cancel anytime by emailing care@hendrxhealth.com; they recommend giving it six months. There is no long-term contract. There is also no published protocol for what happens if the hematocrit climbs, if you want fertility preserved, or what the estrogen inhibitor is. Ask.

    They will not tell you, in public, whether Thursday in C is a real chair, which Labcorp you should use in this zip, or whether the $350 and the $395 are still the numbers on the day you book.

    How it feels in the room

    Ten people have left Google reviews in the set this page is built on, 5.0 stars, all of them fives, from February 16, 2023 through March 13, 2024. Later listings already show thirteen. There is nothing in the polite middle because there is almost nothing in the pile. Perfect scores on a ten-review brand are easy to collect and hard to trust. The useful part is not the 5.0. It is what people bother to mention twice.

    They mention remote. J. H. spelled him Hendrix and still said do not worry about working with someone remotely, that he is easier to get a hold of than a primary-care doctor. They mention availability. B. W. said he had never had a physician who made himself available like that. They mention a young man who had been shrugged off for low T. They mention energy, drive, mood, body, attitude. They mention Tyler by first name. They will not tell you the wait on a Tuesday, whether Thursday is actually open at 10, what the consult costs now if the site moved, or whether you sat in C or on Zoom.

    Healthgrades is a different pile. Sixty-five reviews at 4.6, mixing 2020 flu and COVID primary care, a Rugiet Health note, and early Hendrx patients who liked that the medication showed up in the mail. That 4.6 is not this Google 5.0. Do not merge them.

    The parts that will waste a trip if you skip them
