When can I increase my Wegovy dose?

At the earliest, after 4 weeks at the dose you are on. Wegovy injection Table 1 runs 0.25 mg in weeks 1 through 4, 0.5 mg in weeks 5 through 8, 1 mg in weeks 9 through 12, 1.7 mg in weeks 13 through 16 and the maintenance dosage from week 17 onward. Starting Mon, Jan 5, 2026 that puts 2.4 mg no earlier than Mon, Apr 27, 2026. What happens if I miss two GLP-1 doses in a row?

Why is my tablet schedule in days and my injection schedule in weeks?

Because the labels are written that way, and 30 days is not 4 weeks. Wegovy tablets step on days 1, 31, 61 and 91; Wegovy injection steps on weeks 1, 5, 9, 13 and 17, which are days 1, 29, 57, 85 and 113. After three steps the tablet calendar has drifted six days from the injection one. Converting a tablet schedule into weeks, or an injection schedule into months, is how a step lands on the wrong date.

Can I use this planner for compounded semaglutide or tirzepatide?

No, and the planner will not offer you one. Compounded GLP-1 products are not FDA-approved and carry no approved labelling, so there is no labelled escalation schedule to place on a calendar and nothing to check a compounded titration against. Every one of the seven schedules here is transcribed from a current Structured Product Label on DailyMed. If you have been given a compounded titration plan, the dose and the interval came from whoever wrote it, not from an approved label.

Does delaying an escalation mean I stop the medication?

No. The Wegovy instruction is "If patients do not tolerate a dose during dosage escalation, consider delaying dosage escalation for 4 weeks." — you keep taking the dose you are currently on for another 4 weeks, so that window lengthens rather than a gap appearing in the calendar. Stopping for two or more consecutive weekly doses is the separate situation the label handles by reinitiating escalation at a lower dosage. Why this is a list of earliest dates and not a schedule

Every GLP-1 label in this planner words its escalation interval as a minimum. Ozempic increases the dosage "after at least 4 weeks on the 0.5 mg dosage". Mounjaro and Zepbound increase "in 2.5 mg increments after at least 4 weeks on the current dose". Rybelsus moves at day 31 and again "on Day 61 or thereafter". The word doing the work in all of those is "at least", and it has no upper bound.

That matters because the calendar most people are handed reads like a train timetable, and someone who is vomiting at 1 mg in week 12 reads a printed "week 13: 1.7 mg" as a deadline he is failing. It is not one. Staying where you are is permitted by every label here, indefinitely, and two of the labels name doses below the top of the ladder as maintenance in their own right — Wegovy at 1.7 mg, Zepbound at 5 mg and 10 mg.

Only two of the seven products escalate on a calendar at all. Wegovy injection prints week windows in Table 1 and Wegovy tablets print day windows in Table 2, and both run to maintenance without a condition attached. Ozempic injection, Rybelsus, Ozempic tablets and Mounjaro gate every step above the first on whether additional glycaemic control is needed, so for those four the dates below are the earliest a step could begin if it is taken at all, and most patients never reach the top.

The other half of the job is what happens when the plan does not survive a bad month. The Wegovy injection label answers that twice: "If patients do not tolerate a dose during dosage escalation, consider delaying dosage escalation for 4 weeks." and, after missed doses, "If 2 or more consecutive doses of WEGOVY injection are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions". The planner models both. It also declines to fill in what those instructions leave open — the label does not say which lower dosage to reinitiate at, and this page will not choose one for you.

The minimum holds are the label's: 4 weeks per step for Ozempic, Wegovy injection, Mounjaro and Zepbound, and 30 days per step for all three tablets. A delay lengthens the window you are already in rather than opening a gap, because the label tells you to stay on the dose you are on, not to stop.

A Wegovy injection start on Mon, Jan 5, 2026, taken every Mon, where 1 mg is not tolerated and the step to 1.7 mg is delayed by 4 weeks:

Every date is the earliest the label allows that step, never a date anyone is obliged to hit. Ozempic and Mounjaro gate each escalation on "if additional glycemic control is needed"; Wegovy escalates to maintenance on the calendar and Zepbound escalates once to 5 mg and then in 2.5 mg increments. Thirty days is not four weeks: a 30-day step is two days longer than a 28-day one, and after three steps the tablet calendar has drifted six days from the injection calendar. The label states that Rybelsus and Ozempic tablets "are not substitutable on a mg-to-mg basis", so the columns are three separate ladders — 7 mg of Rybelsus sits opposite 4 mg of Ozempic tablets, not 7 mg. Rybelsus 3 mg and Ozempic tablets 1.5 mg are labelled as not effective for glycemic control; they exist to get you onto the drug.

A delay does not pause the drug: you stay at the dose you are on for four more weeks, so the previous window lengthens rather than a gap appearing. Delaying every escalation moves the recommended 2.4 mg maintenance dosage from Mon, Apr 27, 2026 to Mon, Aug 17, 2026, which is 16 weeks later. The 4-week figure is Wegovy injection's; Ozempic, Rybelsus, Ozempic tablets, Mounjaro and Zepbound carry no delay instruction, and the Wegovy TABLET section says to consider delaying without ever saying by how long — so no number is printed for it here.

Read down a row, never across: the week column and the day column measure different things, and Wegovy's 25 mg tablet and Ozempic's 2 mg injection are separated by more than an order of magnitude in milligrams. Mounjaro names no maintenance dosage at all — section 2.1 escalates only "if additional glycemic control is needed" — so 2.5 mg once weekly can be where a patient stays, which is why its earliest-maintenance cell reads Week 1. Wegovy injection's 7.2 mg maximum is the conditional high dose, reachable no earlier than week 21 and only for weight reduction in adults who have tolerated 2.4 mg for at least 4 weeks.
