An approved label, and the number the trial paper will make you get wrong

Orforglipron is an oral, small-molecule GLP-1 receptor agonist, and the first question to settle about any drug on this site is whether it is approved — because an entry on DailyMed is not an approval. Building our retatrutide chart we found ten listings from a single cross-border e-commerce registrant, every one of them marketing category "Export only", one of them carrying a four-step titration schedule that appears in no published trial. So this page started by reading the approval block. Orforglipron has one SPL, set id 8ac446c5-feba-474f-a103-23facb9b5c62, titled FOUNDAYO (ORFORGLIPRON) TABLET, FILM COATED, registered to Eli Lilly and Company. Its approval block reads marketing category NDA, application NDA220934. That is a real FDA approval with a full prescribing information document, so unlike the retatrutide page this one prints a dose.

The escalation is section 2.1, and it is short. "The starting dosage is 0.8 mg orally once daily. After at least 30 days on the 0.8 mg dosage, increase the dosage to 2.5 mg once daily. After at least 30 days on the 2.5 mg dosage, increase the dosage to 5.5 mg once daily. The dosage may be increased to the next dosage level (9 mg, 14.5 mg, or 17.2 mg once daily) after at least 30 days on the current dosage, based on treatment response and tolerability. The maximum dosage of FOUNDAYO is 17.2 mg once daily." Six strengths, five steps, and the same interval at every one of them.

Read the wording of that interval carefully, because it governs every date this tool produces. It is "after AT LEAST 30 days" — a minimum with no upper bound. Nothing in the label obliges anyone to move on day 31, and past 5.5 mg nothing obliges anyone to move at all: the last three steps are gated on "treatment response and tolerability", which is a clinical judgement rather than a calendar. So every date below is the earliest the label permits that dose, never a scheduled one, and the field the calculator returns is named that way so it cannot be rendered as anything else.

Note also what section 2.1 does not contain. There is no maintenance dosage. Semaglutide's label names one, tirzepatide's names a set, and orforglipron's names neither — it gives a starting dosage of 0.8 mg, a maximum of 17.2 mg, and leaves everything between them as somewhere a person can legitimately stop. Any page telling you the "usual dose" of orforglipron has made that up.

Now the thing this page exists for. The trial that made orforglipron famous is ATTAIN-1, published in the New England Journal of Medicine in 2025, and its abstract reads "once-daily orforglipron at doses of 6 mg, 12 mg, or 36 mg". None of those three numbers is an approved FOUNDAYO strength, and 36 mg is more than twice the approved maximum. The label explains why in section 14: the trials used "an investigational orforglipron formulation", and the label "presents efficacy data from administration of the investigational orforglipron formulation shown as equivalent dosages of once daily FOUNDAYO". The paper's 36 mg arm is the label's 17.2 mg column — you can check it, because the two documents report the same three weight results to a tenth of a percentage point. Quote the paper's dose as an orforglipron dose and you have doubled the ceiling.

One more dosing rule worth knowing, because it moves the ceiling downward. Section 2.2 says the maximum is 9 mg once daily, not 17.2 mg, when a strong CYP3A4 inhibitor is taken alongside — and to avoid strong CYP3A4 inhibitors that also inhibit OATP1B altogether. The calculator carries a switch for that, and when it is on the two strengths above 9 mg are flagged rather than deleted: the label caps the dosage, it does not abolish the tablets.

And the missed-dose rule, which is stricter than the injectables people will be comparing it to. Wegovy reinitiates escalation after two consecutive missed weekly doses; FOUNDAYO after 7 or more consecutive missed daily doses — a week of tablets. What the label does not say is which dose to restart at. "Reinitiate dosage escalation at a lower dosage" names no rung, so this tool lists every lower strength as a candidate and states plainly that the choice is not in the label.

The formula

FOUNDAYO (orforglipron) tablets — NDA 220934, section 2.1

Day 1–30 0.8 mg once daily starting dosage

Day 31–60 2.5 mg once daily directed

Day 61–90 5.5 mg once daily directed

Day 91–120 9 mg once daily if response and tolerability allow

Day 121–150 14.5 mg once daily if response and tolerability allow

Day 151+ 17.2 mg once daily maximum dosage

earliest date for step n = start date + 30 × (n − 1) days

earliest date at the maximum = start date + 150 days

With a strong CYP3A4 inhibitor: maximum 9 mg once daily.

Trial doses are NOT label doses:

NEJM 6 mg → FOUNDAYO 5.5 mg NEJM 12 mg → FOUNDAYO 9 mg

NEJM 36 mg → FOUNDAYO 17.2 mg

Every interval on this label is a minimum — "after at least 30 days on the current dosage" — so the day numbers above are the earliest the label allows a step, not a timetable. The three highest steps are additionally conditional on treatment response and tolerability, which is a clinical judgement the arithmetic knows nothing about. The label names no maintenance dosage at all, so no rung here is "the" dose.

Worked example

A first tablet on Mon, Jan 5, 2026, escalating at the fastest pace the label permits:

  • 0.8 mg — earliest Mon, Jan 5, 2026 (Days 1–30, directed by the label)
  • 2.5 mg — earliest Wed, Feb 4, 2026 (Days 31–60, directed by the label)
  • 5.5 mg — earliest Fri, Mar 6, 2026 (Days 61–90, directed by the label)
  • 9 mg — earliest Sun, Apr 5, 2026 (Days 91–120, based on treatment response and tolerability)
  • 14.5 mg — earliest Tue, May 5, 2026 (Days 121–150, based on treatment response and tolerability)
  • 17.2 mg — earliest Thu, Jun 4, 2026 (Day 151 onward, based on treatment response and tolerability)
  • The maximum dosage is therefore reachable no sooner than 150 days after the first tablet, which is Thu, Jun 4, 2026 — a little over five months, and only if every one of the three conditional steps is taken at the first opportunity.
  • On a strong CYP3A4 inhibitor the same start date stops at 9 mg, reachable no sooner than Sun, Apr 5, 2026, and the 2 strengths above it are off the table while that interaction stands.

Section 2.1, verbatim: "The starting dosage is 0.8 mg orally once daily. The maximum dosage of FOUNDAYO is 17.2 mg once daily." Note the break after 5.5 mg: the first two increases are stated as instructions and the last three as "may be increased … based on treatment response and tolerability". Note also what is missing — the label names no maintenance dosage and no recommended dosage, so any of the four strengths from 5.5 mg upward can be a destination. Section 2.2, on the ceiling in the last column: "Avoid strong CYP3A4 inhibitors that also inhibit OATP1B when taking FOUNDAYO. The maximum dosage of FOUNDAYO is 9 mg once daily when used concomitantly with a strong CYP3A4 inhibitor". The same section handles the other direction of the interaction and does NOT give a number for it: "Avoid strong CYP3A4 inducers when taking FOUNDAYO. Monitor FOUNDAYO effectiveness when concomitantly using moderate CYP3A4 inducers and escalate FOUNDAYO dosage as needed" — escalate as needed, with no modified ladder stated, so none is drawn here.

Earliest permitted, never scheduled. "After at least 30 days" has no upper bound, and nothing in the label obliges a step on day 31. Dates are computed in UTC and formatted from fixed English tables so that the chart, the calculator and a printout all agree. A titration taken at the fastest permitted pace puts the maximum dosage 150 days after the first tablet; a slower one, or a stop at 5.5 or 9 mg, is equally consistent with this label.

Trial 1 enrolled 3,127 adults with obesity or overweight with a comorbidity and excluded type 2 diabetes; Trial 2 enrolled 1,613 adults with a BMI of 27 or more AND type 2 diabetes — which is why its figures are lower at every dose, and why the two trials must not be read as one. Both are 72-week trials and week 72 is the only timepoint the label reports numerically: the time courses are shown in Figures 1 and 2 with no intermediate numbers attached, so none are given here and none have been read off the plots. On the missing data: "The intent-to-treat population consists of all randomized patients. For Trial 1 at Week 72, the body weight endpoint was missing for 24%, 16%, 14%, and 15% of patients randomized to placebo, FOUNDAYO 5.5 mg once daily, 9 mg once daily and 17.2 mg once daily, respectively."

Trial dose against approved strength — the number not to confuse

The published trial and the approved label do not use the same figures. This is the label’s own mapping, and it is the only place on this page the two meet.

Why the two columns differ, in the label's words: "The effectiveness of FOUNDAYO has been established in combination with a reduced-calorie diet and increased physical activity to reduce excess body weight and maintain weight reduction long term in adults with obesity or adults with overweight in the presence of at least one weight-related comorbid condition based on adequate and well-controlled trials of an investigational orforglipron formulation (Trials 1 and 2), referred to in this section as FOUNDAYO. This section of labeling presents efficacy data from administration of the investigational orforglipron formulation shown as equivalent dosages of once daily FOUNDAYO". So the paper's 36 mg arm is this label's 17.2 mg column — and the check is that both documents report the same three results, −7.4/−7.5%, −8.3/−8.4% and −11.1/−11.2%, to within a tenth of a point. Three of the six approved strengths were never maintained doses in either trial: 0.8 mg, 2.5 mg and 14.5 mg exist to get a patient up the ladder, and no efficacy figure is published for them. If you see "orforglipron 36 mg" quoted as a dose anywhere, it is the trial formulation, and it is more than twice the approved maximum of 17.2 mg.

Frequently asked questions

Is orforglipron FDA approved?
Yes. It is approved as FOUNDAYO (orforglipron) film-coated tablets, marketed by Eli Lilly and Company under application NDA220934, and its DailyMed SPL (set id 8ac446c5-feba-474f-a103-23facb9b5c62) carries the marketing category "NDA" in its approval block. That last detail is worth checking yourself on any drug: a DailyMed listing on its own proves nothing, and we have twice found SPLs on this site that looked like labels and were not — ten retatrutide listings filed "Export only" by a cross-border e-commerce registrant, and a topical finasteride self-registered under an OTC monograph. FOUNDAYO is the real thing: an NDA with a boxed warning about thyroid C-cell tumors and a full prescribing information document.
What is the starting dose of orforglipron?
Section 2.1 of the FOUNDAYO label states it directly: "The starting dosage is 0.8 mg orally once daily." It is taken with or without food, swallowed whole — "Do not break, crush, or chew" — and no more than one tablet a day. The label then directs two increases: to 2.5 mg after at least 30 days on 0.8 mg, and to 5.5 mg after at least 30 days on 2.5 mg. Everything above 5.5 mg is conditional rather than directed.
What is the maximum dose of orforglipron?
"The maximum dosage of FOUNDAYO is 17.2 mg once daily" — section 2.1. That figure drops to 9 mg once daily when a strong CYP3A4 inhibitor is taken alongside, per section 2.2, which also says to avoid strong CYP3A4 inhibitors that additionally inhibit OATP1B. At the fastest pace the label permits, 17.2 mg cannot be reached sooner than 150 days after the first tablet, because there are five steps and each requires at least 30 days on the dose below.
Why does the trial say 36 mg when the label says 17.2 mg?
Because they are two different formulations of the same molecule. The phase 3 trials used an investigational formulation, and section 14 of the label says it "presents efficacy data from administration of the investigational orforglipron formulation shown as equivalent dosages of once daily FOUNDAYO". So the NEJM paper's 6, 12 and 36 mg arms appear in the label as 5.5, 9 and 17.2 mg, and you can verify the pairing because both documents report the same three week-72 weight results to within a tenth of a percentage point. Treating 36 mg as an orforglipron dose overstates the approved maximum by more than two-fold, and it is the single likeliest error on this subject.
How long does it take to reach the full orforglipron dose?
No fewer than 150 days — five steps at a minimum of 30 days each. Starting on Mon, Jan 5, 2026, the earliest permitted dates are Wed, Feb 4, 2026 for 2.5 mg, Fri, Mar 6, 2026 for 5.5 mg, Sun, Apr 5, 2026 for 9 mg, Tue, May 5, 2026 for 14.5 mg and Thu, Jun 4, 2026 for 17.2 mg. Those are floors, not a schedule: "after at least 30 days" has no upper limit, and the last three steps happen only "based on treatment response and tolerability". Plenty of people will stop lower and still be following the label.
What is the maintenance dose of orforglipron?
The label does not name one, and this page will not either. Semaglutide's label names a recommended maintenance dosage and tirzepatide's names a set of them; section 2.1 of the FOUNDAYO label names a starting dosage of 0.8 mg and a maximum of 17.2 mg and nothing in between is labelled as a destination. In practice that means any of 5.5, 9, 14.5 or 17.2 mg can be where someone stays, and the label leaves the choice to response and tolerability. Anyone quoting you the "usual" or "typical" maintenance dose of orforglipron is supplying a figure the label does not contain.
What happens if I miss a week of orforglipron?
Section 2.3 is specific: "If a dose is missed, instruct patients to take the dose as soon as possible. Advise patients not to double up the next dose. If 7 or more consecutive doses are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions". Note that seven consecutive daily doses is one week, and that the rule is about consecutive doses rather than total missed. What the label does not say is WHICH lower dosage. For someone on 14.5 mg the lower approved strengths are 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, and the label names none of them — the choice is your prescriber's, not this calculator's.
How much weight did people lose on orforglipron?
At week 72, in the label's Trial 1 — 3,127 adults with obesity or overweight with a comorbidity, without diabetes — the mean change in body weight was −7.4% at 5.5 mg, −8.3% at 9 mg and −11.1% at 17.2 mg, against −2.1% on placebo. At 17.2 mg, 71.5% lost at least 5% of their body weight, 54.5% at least 10%, 35.9% at least 15% and 18.4% at least 20%; on placebo those figures were 26.8%, 13%, 6% and 2.9%. In Trial 2, in 1,613 adults who also had type 2 diabetes, every figure was lower — −9.6% at 17.2 mg. Those are population means at one fixed timepoint in two defined populations, not a forecast for an individual.
Is orforglipron dosing affected by kidney or liver problems?
The label answers both and they differ. Section 8.7: "No dosage modification of FOUNDAYO is recommended for patients with renal impairment", and it adds that no change in pharmacokinetics was seen even in end-stage renal disease — though it says to monitor renal function in patients with adverse reactions that could cause volume depletion. Section 8.6 is stricter on the liver: no dosage modification in mild or moderate hepatic impairment (Child-Pugh A and B), but FOUNDAYO "is not recommended for use in patients with severe hepatic impairment (Child-Pugh Class C)". Neither of those is a dose this calculator can compute — they are decisions about whether to prescribe at all.

Every interval on that label is a minimum and every step above 5.5 mg is conditional on response and tolerability, so no date here is a schedule. Selecting a dose, moving up a step, holding, or restarting after missed doses are your prescriber’s decisions.
