Wegovy
A weekly injection that titrates to 2.4 mg and may go to 7.2 mg, and a daily tablet that ends at 25 mg — two medicines, one brand name, and only one of them treats a liver disease or a twelve-year-old.

Why "Wegovy" does not identify a treatment
Wegovy is two separately approved products described in a single prescribing information: WEGOVY injection under NDA215256 and WEGOVY tablets under NDA218316. They are not two strengths of one thing. The injection is given once a week under the skin and escalates over sixteen weeks to a maintenance dose of 2.4 mg, with a further step to 7.2 mg available for weight reduction in adults who tolerate 2.4 mg. The tablet is swallowed every morning on an empty stomach and escalates over ninety days to 25 mg. Neither number means anything without knowing which of the two is in the box.
The indications do not match either. The injection is approved for cardiovascular risk reduction, for weight reduction in adults and in patients aged twelve and older, and — alone among every semaglutide product sold in the United States — for noncirrhotic MASH with moderate to advanced liver fibrosis, under accelerated approval. The tablet carries the first two of those, adults only. A man told "you are on Wegovy" for a liver indication is necessarily on the injection, because the tablet is not approved for it.
None of this is a property of semaglutide. The same molecule is sold as Ozempic injection, which stops at 2 mg weekly and is not approved for weight at all, as Ozempic tablets at 9 mg daily and as Rybelsus at 14 mg daily. Five products, five ladders, four ceilings that differ by up to 3.6× within a single route. The table below is the part of that story that belongs to this brand.
What is true of Wegovy and not of semaglutide
Each row is something a reader would reasonably assume belongs to the molecule, and does not. These are the facts that change when the brand name on the carton changes, while the drug inside it does not.
| Attribute | Wegovy | Elsewhere, same molecule |
|---|---|---|
| Maximum labelled doseSemaglutide has no maximum dose; five approved products carry five ceilings. The highest weekly injectable one is 3.6× the Ozempic ceiling and the highest oral one is 2.8× the Rybelsus ceiling, so a milligram figure quoted without a brand cannot be checked against anything. | Injection: 2.4 mg once weekly, rising to 7.2 mg once weekly for weight reduction in adults only. Tablets: 25 mg once daily. | Ozempic injection stops at 2 mg once weekly, Ozempic tablets at 9 mg once daily, Rybelsus at 14 mg once daily. |
| A liver indication no other semaglutide product hasThe one indication most likely to be described as "semaglutide for fatty liver" belongs to exactly one presentation of one brand. Someone prescribed the tablet for it is outside the label of the product he is holding, even though the carton says Wegovy. | Wegovy injection is indicated for noncirrhotic MASH with moderate to advanced liver fibrosis (stages F2 to F3) in adults, under accelerated approval. | No other semaglutide product carries it — and neither do Wegovy tablets, which share this label and this brand name but not this indication. |
| Who the product may be given toOne molecule, one age group, three different labelled positions decided by the carton. The paediatric approval also does not extend to the 7.2 mg dosage or to anyone under twelve, so "approved for teenagers" is true of one ladder and not of its top step. | Injection: adults, and patients aged 12 years and older with obesity for weight reduction. Tablets: adults only, with pediatric safety and effectiveness not established. | Ozempic states flatly that safety and efficacy have not been established in pediatric patients, for every one of its indications and both of its forms. |
| What the pen isThe commonest Wegovy device has no dial and cannot be set to a wrong dose, because the dose is the pen. The FlexTouch can, which is why only the FlexTouch carries the label’s never-share warning and the 56-day in-use limit. Advice written for one is wrong for the other. | The standard Wegovy pens and syringes are SINGLE-dose: one device, one 0.25 to 2.4 mg injection, discarded after use, and the 7.2 mg WEGOVY HD is its own single-dose pen. The FlexTouch is a separate product — a single-patient-use pen holding 9.6 mg/3 mL (3.2 mg/mL), four weekly 2.4 mg doses. | Every Ozempic pen is a multi-dose dial-a-dose device — 2 mg/3 mL, 4 mg/3 mL or 8 mg/3 mL — that the patient sets a dose on before each injection. |
| How long it lasts out of the fridgeTwo Wegovy presentations in the same fridge have different discard rules, and the 56-day figure people repeat belongs only to the four-dose pen. A single-dose pen left out is counted from a different clock and thrown away after one injection regardless. | Single-dose pen or syringe: refrigerated, and if needed 8 °C to 30 °C for up to 28 days before the cap is removed, then discarded after use. FlexTouch: 56 days at 15 °C to 30 °C after first use. | Ozempic pens are 56 days after first use; the Ozempic single-dose prefilled syringe is 28 days at 8 °C to 30 °C; Wegovy tablets have no cold chain at all and sit at room temperature in their bottle. |
| What 9 mg meansThe first three rungs of the two oral ladders are numerically identical — 1.5 mg, 4 mg, 9 mg — and then one keeps going. A man told to stay at 9 mg and a man told to move up from 9 mg are both following their own label correctly. | On a Wegovy tablet, 9 mg is days 61 to 90 — a thirty-day rung on the way up to 25 mg. | On an Ozempic tablet, 9 mg is the top of the ladder: the dose the label escalates to and stops at, reached from day 61 onward. |
| Whether the tablet and the injection give the same exposureThis is the only place in the semaglutide labelling where one brand tells you its own oral and injectable forms are not equivalent in a named group of patients. The label’s own remedy is to consider switching to the injection, not to raise the tablet. | Section 8.6 states that in patients without type 2 diabetes the average blood concentrations from 25 mg tablets and 2.4 mg weekly injection are similar, and that in patients WITH type 2 diabetes the tablet’s average concentrations are lower — so some may experience subtherapeutic concentrations. | The Rybelsus and Ozempic tablet label makes the comparison in the opposite direction, stating only that the two oral brands "are not substitutable on a mg-to-mg basis" and giving named dose pairings. |
| What switching is authorisedNo label anywhere gives a general milligram equivalence between a semaglutide injection and a semaglutide tablet. Every authorised switch is a named dose to a named dose, and nothing in between has been evaluated. | Only between 2.4 mg injection and 25 mg tablets, in both directions, at named intervals — one week after stopping the injection, or the day after stopping the tablets. | The Ozempic and Rybelsus labels authorise a different, non-overlapping set: 0.5 mg injection to 4 mg or 9 mg Ozempic tablets, 7 mg Rybelsus to 4 mg Ozempic tablets, 14 mg Rybelsus to 9 mg Ozempic tablets. |
Boxed warning — risk of thyroid C-cell tumors
In rodents, semaglutide causes dose-dependent and treatment-duration-dependent thyroid C-cell tumors at clinically relevant exposures. It is unknown whether WEGOVY causes thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in humans as human relevance of semaglutide-induced rodent thyroid C-cell tumors has not been determined.

What this product is approved to treat
- —to reduce the risk of major adverse cardiovascular (CV) events (CV death, non-fatal myocardial infarction, or non-fatal stroke) in adults with established CV disease and either obesity or overweight.
- —to reduce excess body weight and maintain weight reduction long term in: adults and pediatric patients aged 12 years and older with obesity; adults with overweight in the presence of at least one weight-related comorbid condition.
- —for the treatment of noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH), formerly known as nonalcoholic steatohepatitis (NASH), with moderate to advanced liver fibrosis (consistent with stages F2 to F3 fibrosis) in adults. This indication is approved under accelerated approval based on improvement of MASH and fibrosis.
The labelled ladder
| When | Dose | What the label attaches to this step |
|---|---|---|
| Weeks 1–4 | 0.25 mg once weekly | — |
| Weeks 5–8 | 0.5 mg once weekly | — |
| Weeks 9–12 | 1 mg once weekly | — |
| Weeks 13–16 | 1.7 mg once weekly | Also a maintenance option where 2.4 mg is not tolerated. |
| Week 17 onward | 2.4 mg once weekly | — |
| Week 21 onward | 7.2 mg once weekly | Not in Table 1. The label gates this on tolerance — weight reduction in adults only, after tolerating 2.4 mg for at least 4 weeks — and on additional weight reduction being clinically indicated. Week 21 onward is week 17 plus the 4 weeks at 2.4 mg the label requires; the label itself prints no week number for this step. |
Ceiling. The label's own words for the top step: "For patients who tolerate the 2.4 mg dosage for at least 4 weeks and additional weight reduction is clinically indicated, the dosage may be increased to a maximum dosage of 7.2 mg subcutaneously once weekly." That sentence sits under Weight Reduction in Adults and nowhere else — for cardiovascular risk reduction, for paediatric patients and for MASH the label's maintenance dosage stops at 2.4 mg.

Administration, as the label words it
- —Administer WEGOVY injection once weekly, on the same day each week, at any time of day, with or without meals.
- —Inject WEGOVY subcutaneously in the abdomen, thigh, or upper arm. The time of day and the injection site can be changed without the need for a dosage modification.
- —If one dose of WEGOVY injection is missed and the next scheduled dose is: more than 2 days away, administer WEGOVY injection as soon as possible. less than 2 days away, do not administer the WEGOVY injection dose. Resume dosing on the regularly scheduled day of the week.
- —If 2 or more consecutive doses of WEGOVY injection are missed, reinitiate dosage escalation at a lower dosage to reduce the risk of gastrointestinal adverse reactions.
- —WEGOVY FlexTouch must never be shared between patients, even if the needle is changed. Pen-sharing poses a risk for transmission of blood-borne pathogens.

Ceiling. 25 mg orally once daily, which the label names as the recommended maintenance dosage rather than as a ceiling to be reached if needed: "The recommended maintenance dosage of WEGOVY tablets is 25 mg orally once daily." There is no higher oral step, and no step above it in any semaglutide label.

The same molecule under other brand names
Everything below is semaglutide. None of it is Wegovy, and a dose from one column carried into another is not a smaller or larger version of the same instruction — it is a different product’s schedule.
| Brand | Form | Approved for | Ladder | Maximum |
|---|---|---|---|---|
| Ozempic injection | Subcutaneous, once weekly | Type 2 diabetes — glycaemic control; CV risk reduction in type 2 diabetes with established CV disease; chronic kidney disease in type 2 diabetes | 0.25 mg → 0.5 mg → 1 mg → 2 mg | 2 mg once weekly |
| Ozempic tablets | Oral, once daily | Type 2 diabetes — glycaemic control, and CV risk reduction in type 2 diabetes at high risk | 1.5 mg → 4 mg → 9 mg | 9 mg once daily |
| Rybelsus tablets | Oral, once daily | Type 2 diabetes — glycaemic control, and CV risk reduction in type 2 diabetes at high risk | 3 mg → 7 mg → 14 mg | 14 mg once daily |
Switching between products
Only the top dose of one is authorised to become the top dose of the other
Patients taking WEGOVY 2.4 mg injection for cardiovascular risk reduction or weight reduction in adults may switch to WEGOVY 25 mg tablets. One week after discontinuing WEGOVY 2.4 mg injection, initiate 25 mg of WEGOVY tablets orally once daily.
Wegovy injection to Wegovy tablets
The return trip has a different gap and a tolerance escape hatch
The day after discontinuing WEGOVY tablets 25 mg once daily, initiate WEGOVY 2.4 mg subcutaneous injection once weekly. For patients who do not tolerate WEGOVY 25 mg tablets, consider switching to WEGOVY 1.7 mg injection.
Wegovy tablets to Wegovy injection
Not with each other, and not with anything else in the class
Concomitant use of WEGOVY (semaglutide) tablets or WEGOVY (semaglutide) injection with other semaglutide-containing products or with any other GLP-1 receptor agonist is not recommended.
Wegovy injection and Wegovy tablets
These are the only switches the labels name. There is no general milligram equivalence between one product and another, and this page does not supply one.
| From | To | When |
|---|---|---|
| Wegovy injection, 2.4 mg once weekly | Wegovy tablets, 25 mg once daily | One week after discontinuing the injection |
| Wegovy tablets, 25 mg once daily | Wegovy injection, 2.4 mg once weekly | The day after discontinuing the tablets |
What this page does not tell you
- Price — list, cash or copay
- No price appears anywhere on this page, and no price is emitted in its structured data. Wegovy pricing varies by presentation, pharmacy, insurance design and manufacturer programme, and we could not find a source for a current US figure that we could cite and re-verify the way every other number here is cited. A stale or wrong price is the most consequential thing a page like this could print, so the field is absent rather than estimated.
- The BMI you need to qualify
- Section 1 of the Wegovy label contains no kg/m² figure at all, for either product. It says "obesity" and "overweight in the presence of at least one weight-related comorbid condition" and defines neither numerically. Every BMI threshold circulating for Wegovy — 30, 27, 35 — is an enrolment criterion from a clinical trial in section 14, not an eligibility rule in the indication. For a reader working out whether he qualifies that is a materially different claim, so this page does not print one.
- How many insulin-syringe units a dose equals
- The question does not apply to any Wegovy presentation. The standard pens and syringes are single-dose devices pre-filled at one strength, the WEGOVY HD pen delivers exactly 7.2 mg, and the FlexTouch dials in milligrams. There is no vial to draw from and no unit mark to read. Any "Wegovy units chart" is describing a compounded product, which is a different medicine with no approved labelling.
- How much weight you should expect to lose
- The label reports weight-reduction results from named trials in named populations over fixed periods, and those results are not a forecast for an individual. Reproducing a mean percentage here, stripped of the trial it came from and of the diet and activity programme every arm was on, would read as a promise the label does not make. The trial results are in section 14 of the SPL linked below, with their populations attached.
Frequently asked questions
- Is Wegovy a weekly injection or a daily tablet?
- Both. WEGOVY injection (NDA215256) is given once weekly under the skin and titrates over 16 weeks from 0.25 mg to a 2.4 mg maintenance dose. WEGOVY tablets (NDA218316) are swallowed every morning on an empty stomach and titrate over 90 days from 1.5 mg to 25 mg. They share one prescribing information and one boxed warning, and they are different medicines with different ceilings, different indications and different devices.
- What is the maximum dose of Wegovy?
- For the injection the label states: "For patients who tolerate the 2.4 mg dosage for at least 4 weeks and additional weight reduction is clinically indicated, the dosage may be increased to a maximum dosage of 7.2 mg subcutaneously once weekly." That 7.2 mg step exists only under Weight Reduction in Adults — for cardiovascular risk reduction, for patients aged 12 and older and for MASH, the maintenance dosage stops at 2.4 mg. For the tablets the recommended maintenance dosage is 25 mg once daily, and there is no step above it.
- When can someone reach the 7.2 mg dose?
- Not before week 21, and only if every step is taken at the minimum interval. Table 1 puts 2.4 mg at "17 and onward", and the 7.2 mg sentence requires tolerating 2.4 mg for at least 4 weeks — 17 plus 4. The label itself prints no week number for the 7.2 mg step; that figure is the sum of two figures it does print, and there is no upper bound on how long a step may be held.
- What BMI do you need for Wegovy?
- The label does not say. Section 1 of the Wegovy prescribing information contains no kg/m² figure for either the injection or the tablets — it says "obesity", or "overweight in the presence of at least one weight-related comorbid condition", and leaves both undefined. The numbers people quote, typically 30 or 27, come from the enrolment criteria of the trials in section 14. Whether an individual qualifies is a prescriber’s judgement against an indication that carries no number.
- Is the Wegovy pen reusable?
- Usually not. The standard Wegovy pens and the prefilled syringes are single-dose: one device holds one injection of 0.25, 0.5, 1, 1.7 or 2.4 mg and is discarded after use, and the 7.2 mg WEGOVY HD is its own single-dose pen. The exception is WEGOVY FlexTouch, a single-patient-use pen holding 9.6 mg/3 mL (3.2 mg/mL) — four weekly 2.4 mg doses — which is the only Wegovy presentation with a 56-day in-use limit and the only one carrying the label’s warning never to share a pen between patients.
- Is Wegovy the same as Ozempic?
- Same molecule, different products. Ozempic injection caps at 2 mg once weekly and is approved for type 2 diabetes, cardiovascular risk and chronic kidney disease — not for weight. Wegovy injection reaches 2.4 mg and may go to 7.2 mg, and is approved for weight reduction, cardiovascular risk reduction in obesity or overweight, and noncirrhotic MASH. Neither label authorises substituting one for the other, and no label anywhere gives a milligram equivalence between them.
- Can you switch between Wegovy tablets and the injection?
- Only at the doses the label names. Going oral: one week after discontinuing WEGOVY 2.4 mg injection, start 25 mg tablets once daily. Coming back: the day after discontinuing 25 mg tablets, start 2.4 mg injection once weekly, or 1.7 mg for someone who did not tolerate the tablets. No other pairing is authorised, and section 8.6 warns that in patients with type 2 diabetes the tablet produces lower average blood concentrations than the injection.
- What happens if a weekly Wegovy injection is missed?
- The label distinguishes by how close the next dose is: if it is more than 2 days away, take the missed dose as soon as possible; if it is less than 2 days away, skip it and resume on the regular day. If 2 or more consecutive doses are missed, the label directs reinitiating dosage escalation at a lower dosage. The tablet rule is simpler and different — skip the missed dose and take the next one the following day.
